Abstract: Diabetes mellitus is a common medical condition complicating pregnancy with poor outcome. Early diagnosis and proper blood glucose control may reduce complications. The incidence and outcomes of the disease in Irrua is not known. The aim of this study is to determine the incidence and outcomes of diabetes mellitus complicating pregnancy in Irrua. We carried out a six year retrospective review of the demographic characteristics of the patients, parity, clinical and booking status, number of hospital admissions for blood sugar control and pregnancy outcome of diabetes mellitus complicating pregnancy in Irrua Specialist Teaching Hospital, Irrua between January 1, 2000 and December 31, 2005. Data collected were analysed using SPSS window version 20.0 statistical package. In the study period, there were 5,460 pregnant women who booked for antenatal care in the hospital, out of these, 2,765 attended the antenatal carefully and eventually delivered in the hospital and 30 of them whose pregnancies were complicated with diabetes mellitus were studied. The incidence of diabetes mellitus complicating pregnancy in this study is 10.85 pregnancies per 1000 births. The mean age of the patients was 33 years. Multipara were affected in 62% of cases. Gestational diabetes predominated in 66.7% of cases. There were family history of diabetes in 73.3% of cases. All patients had both dietary and insulin therapy throughout antenatal period. Majority of the patients were admitted for blood glucose control once. Majority of the patients (76.6%) had vaginal deliveries while 23.3% had caesarian deliveries due to fetal and maternal complications. Over 73% of the patients delivered at term. The outcome of pregnancy was poor. The incidence of diabetes mellitus complicating pregnancy in Irrua is 10.85 pregnancies per 1000 births, this is high with relatively poor outcome. Preconceptional counseling, deliberate blood glucose tolerance testing and adequate blood glucose control in pregnancy is recommended.

Abstract: OBJECTIVE: To investigate the clinical diagnosis, preventive antenatal screening and antenatal diagnosis, pregnancy management, timing and mode of delivery and postnatal management of β-thalassaemia in pregnancy combined. METHODS: Retrospective analysis of the medical history and treatment of a patient with β-thalassaemia in pregnancy combined with pregnancy outcome. RESULTS: The patient recovered well, had a normal temperature, the abdominal incision was removed at 7 d, the II/nail healed, the general obstetric condition was acceptable and she was discharged successfully. CONCLUSION: Anaemia is very common in pregnancy, but very severe anaemia in pregnancy is extremely rare and it is important to define the cause of anaemia. Thalassaemia is a group of inherited chronic haemolytic disorders caused by autosomal defects, which can be aggravated by pregnancy. Thalassaemia can be divided into α-, β-, γ-, δ-, δβ- and other categories, and for β-thalassaemia it can be divided into mild, moderate and severe, with both intermediate and severe patients presenting early with obvious anaemic symptoms and relying on long-term transfusion therapy. The majority of these patients die in childhood and are extremely rare in pregnancy. Thalassaemia in pregnancy can directly affect the outcome of pregnancy and cause many near and long-term complications in the newborn, and timely antenatal screening and prenatal diagnosis can detect the disease early. Active obstetric management and timing of pregnancy can also help to improve maternal and infant pregnancy outcomes.

Abstract: Purpose: Intrauterine fetal death (IUFD) is a problem frequently encountered in obstetrical practice in developing countries. The objectives of this study were to identify maternal risk factors for IUFD in the third trimester of pregnancy and to establish its relationship with the norms of periodicity of antenatal care in Kamina. Materiel and methods: This is a case-control study conducted in 6 health facilities in Kamina from January 2021 to May 2022. The effects of the socio-demographic characteristics and obstetrical history of mothers, number of prenatal attendances, age of pregnancy at the first prenatal attendance and number of consultations in third trimester of pregnancy on the occurrence of IUFD were studied. A total of 78 cases of IUFD were linked to 234 controls. Results: The adjustment allowed us to retain 5 parameters significantly associated with IUFD: residence in a rural environment (aOR=2.25; CI= [1.23-4.48]), absence of a history of abortion (aOR=2.97; CI=[1.21-7.26]), history of stillbirth (aOR=2.97; CI =[1.21-7.26]), non-follow up of prenatal care (aOR=3.69; CI=[1.78-7.68]) and insufficient number of prenatal attendances in the third trimester of pregnancy (aOR= 2.22; CI=[1.04 -4.73]). The values of the area under the ROC curve (AUC) found indicate a good capacity of the model (80.3%). Conclusion: The modifiable maternal determinants and those related to identified ANC should serve providers to reduce the occurrence of IUFD in Kamina and its surroundings.

Abstract: Mature ovarian teratomas are benign germ cell tumors of the ovary, accounting for about 10 % -20 % of ovarian tumors and 95 % of ovarian teratomas. Mature ovarian teratomas are common in young women. They are usually unilateral, slow growing, cystic, mobile, smooth, with little or no ascites. Ovarian cystectomy for benign neoplasms instead of oophorectomy is mandatory for ovarian function preservation. We report a case of a large pelvic teratoma in a 19-year-old adolescent female, approximately 20.7cm×10.5cm in size. The patient's main clinical presentation was mild lower abdominal pain, After transumbilical single-port laparoscopic ovarian cystectomy, the final histopathological report was bilateral ovarian mature teratoma. This case report suggests that transumbilical single-port laparoscopic-assisted in vitro giant ovarian tumor ablation is a safe and feasible surgical approach for patients with benign large adrenal tumors. it effectively combines the advantages of open surgery and laparoscopic resection of large ovarian tumors, which not only protects ovarian function, but also avoids cyst fluid overflow and has significant postoperative cosmetic effects. Nevertheless, this study also had some limitations. First, this was a case report and not a multi-center randomized controlled trial. Second, it included a single patient and the short follow-up data precluded us from deriving any conclusions about the long-term efficacy and safety of this method.

Abstract: Background: Diffuse uterine leiomyomatosis is a benign disease with a low incidence rate, mainly manifested as menorrhagia and infertility. Due to its clinical manifestations similar to multiple uterine myomas or adenomyosis, it is often misdiagnosed in clinical practice. Research has found that they can be distinguished through histopathological examination. According to existing reported cases, Diffuse uterine leiomyomatosis mainly occurs during the child-bearing period and has a significant impact on fertility. Hysterectomy is recognized as a radical cure, but uterine-sparing treatment methods need to be continuously explored. Case: A 41-year-old woman presented with recurrent menorrhagia 5 years after myomectomy. B-ultrasound examination indicated that the uterus was enlarged and the myometrium was full of hypoechoic, Considering the patient's desire to preserve the uterus, we performed laparotomy myomectomy again. Result: Hysteroscopic myomectomy has been shown to have a certain therapeutic effect on infertile patients in the early stage of DUL. Laparotomy myomectomy can remove a wider range of lesions and can be combined with gonadotropin releasing hormone agonists or antagonists for pretreatment. Uterine artery embolization is also a commonly administered therapy. High-intensity focused ultrasound ablation has been used with significant success in reducing uterine volume but requires further evaluation for its impact on fertility. Conclusion: A DUL patient underwent extensive myomectomy. This atudy further discussed uterine-sparing treatments, and reviewed existing research reports.
