Distribution of Cardiovascular Risk Among Diabetic Patients in Kumasi

Abstract: Cardiovascular diseases (CVDs) are the leading cause of mortality globally. Cardiovascular risk scores are reliable tools used to predict an individual’s chance of developing a cardiovascular event. This study assesses the distribution of cardiovascular risk among diabetic patients attending a diabetic clinic in a district hospital in Kumasi. This is a hospital-based cross-sectional study among 94 diabetic patients attending a diabetic clinic in Kumasi, Ghana. Data collected includes sociodemographic information, anthropometry, medical history and lipid profile which were then used to compute the cardiovascular risk score using the pooled cohort equation (PCE) and WHO non-laboratory scoring tools. The average risk score was 13.5% [CI 95: 10.8 – 16.1] according to the PCE tool and 7.2% [CI 95: 6.2 – 8.1] according to the WHO non-laboratory risk scoring tool. The PCE categorised 52.1%, 25.5% and 22.3% as low, moderate and high risk respectively whiles the WHO non-lab categorised 78.7% and 21.3% as low and moderate risk respectively, with no one at high risk. Majority of our study participants were at low risk of developing a cardiovascular event in 10-years according to both tools. There was significant difference between the pooled cohort equation and WHO non-lab risk scoring calculators.

Intraoperative Pericardial Tamponade During Transjugular Intrahepatic Portosystemic Stent-Shunt Surgery

Abstract: Introduction: Portal hypertension is a serious medical condition associated with fatal gastrointestinal bleeding, liver function abnormalities, and other adverse events. Transjugular intrahepatic portosystemic stent-shunt surgery (TIPSS) has emerged as an effective treatment for alleviating portal hypertension due to its advantages of minimal invasiveness, rapid recovery, and significant therapeutic outcomes. However, the perioperative period of TIPSS is not devoid of risks, and serious complications may arise. Objective: This paper aims to present a case of pericardial tamponade resulting from poor puncture during TIPSS, highlighting the potential life-threatening complications that can occur during this procedure. The management of pericardial tamponade in the perioperative setting is crucial for ensuring perfusion of vital organs and the overall well-being of the patient. Methods: A comprehensive review of the patient's medical records, perioperative notes, and diagnostic imaging was conducted to analyze the circumstances leading to the development of pericardial tamponade following TIPSS. Results: We present a case of a 39-year-old male with portal hypertension who underwent TIPSS as a therapeutic measure. During the procedure, a poor puncture led to the unintended occurrence of pericardial tamponade, causing a rapid deterioration of the patient's hemodynamic status. Immediate recognition of the complication by the anesthesiologist allowed for prompt intervention, with pericardiocentesis performed successfully to alleviate the tamponade and restore cardiac function. Conclusion: Pericardial tamponade following TIPSS is a rare but potentially fatal complication that demands immediate recognition and swift management. This case underscores the significance of close perioperative monitoring and the expertise of the medical team in dealing with unforeseen adverse events during TIPSS. Awareness of such complications can aid in early diagnosis and timely intervention, ultimately contributing to favorable patient outcomes.

Abstract: Objective: This study aimed to comprehensively explore the effect of parental collaborative care on mitigating cardiovascular risk factors in adolescents, encompassing the multifaceted collaborations with partners, healthcare professionals, and support networks. Methods: A prospective cohort design was employed, involving 154 parents of adolescents with high cardiovascular risk. Participants were randomized into an intervention group (n=80) and a control group (n=80). The intervention group received collaborative care following the diagnosis of their children's high cardiovascular risk, while the control group parents did not receive any specific intervention. Cardiovascular risk assessment was conducted through a school-based sampling survey, with parental consent obtained. Data collection involved a three-month follow-up period, with telephone interviews conducted to assess parent satisfaction and problem-solving rates. A collaborative care questionnaire was developed to explore parents' experiences in collaborative caregiving. Results: The demographic characteristics of parents in both the intervention and control groups were comparable, with no significant differences observed in gender distribution, age, marital status, education levels, and family incomes. Parents in the collaborative care group reported significantly higher levels of satisfaction and problem-solving rates compared to those receiving common nursing intervention. The most commonly expressed concerns by parents were related to communication, stress management, and parenting tasks. However, control group parents exhibited higher rates of asking questions about problem-solving parenting issues and their children's exams, health, or care plan. Conclusion: Collaborative care was associated with increased parent satisfaction and problem-solving rates among parents of adolescents with high cardiovascular risk. These findings highlight the potential benefits of incorporating collaborative care strategies in the management of pediatric patients with cardiovascular risk and their families. Further research is needed to explore the specific components of collaborative care that contribute to improved outcomes and identify areas for refinement to optimize its effectiveness.

Abstract: Background: Pseudoaneurysm of the coronary artery is a rare condition with an incidence of less than 6%. Only few cases have been reported in the literature. The etiology, treatment options and the use of noninvasive diagnostic tools like Computed tomography need to be discussed for understanding the diagnosis and management of this condition. Case report: We report a rare case of pseudoaneurysm of the left circumflex artery with distal thrombosis following one year after percutaneous coronary intervention in an adult male patient who presented to us as an asymptomatic follow up patient post percutaneous coronary intervention of left circumflex artery for coronary CT angiography. The CT angiogram showed a 2x2 cm pseudoaneurysm with patent proximal stent and distal segment thrombosis with a patent second obtuse marginal artery. We discuss the imaging findings along with the etiology and treatment options of this condition. CONCLUSION: This case highlights the potential risks of coronary stenting especially in complex cases which can result in iatrogenic vessel injury and pseudoaneurysm formation. The case was more challenging as the patient was asymptomatic and there was also distal occlusion of the involved vessel segment. The likely etiology in this case was insertion of prior DES and the treatment options ranged from conservative management with short follow-up, embolization, inserting covered stents and open surgery with aneurysmectomy.
