Contribution of the Triglycerides-Glucose Index (TyG) in the Assessment of Insulin Resistance in Pregnant Women During an OGTT Test

Abstract: The Triglyceride-Glucose (TyG) index is a marker used to assess insulin resistance which is associated with the occurrence of gestational diabetes. The aim of this study is to determine the benefit of the TyG within the diagnosis of insulin resistance in pregnant women in an orally induced hyperglycaemia test. This study was conducted at the Department of Biochemistry, at Aristide le Dantec University Hospital in collaboration with the department of gynaecology. Women with fasting blood glucose < 0.92 g/L at 12 weeks of age benefited from the OGTT 75 test following the WHO protocol. The TyG index was calculated using the formula: Ln [fasting triglyceridemia (mg/dL) × fasting blood glucose (mg/dL)]/2. Data were analysed using SPSS v.26 and a p< value of 0.05 was considered statistically significant. A total of 102 patients were included in the study with a mean age of 29± to 5.9 years. The OGTT 75 test was positive in 24 women (23.5%) with a predominance in the third trimester (19.4%). TyG values were significantly higher in women with gestational diabetes (8.76, ±0.5vs8.2±, 0.5; p<0.0001), in contrast to the HOMA index (6.43, ±16.8, ±1.15±, 0.9; p=0.138). The AUC of the ROC curve for the TyG-H0, TyG-H1 and TyG-H2 indices were 0.808 (95% CI)=0.70-0.92), 0.808 (95% CI)=0.698-0.918), 0.818 (95% CI)=0.70-0.93, respectively. For the HOMA-IR index, the AUC was 0.799 (95% CI = 0.69-0.91). The cut-off value for the TyG-H1 index was 9.5 with a sensitivity and specificity of 79.2% and 71.8%, respectively. The TyG index is a better way to assess the level of insulin resistance in pregnant women than using the HOMA index, especially after the first hour of a glucose load of 75g.

Abstract: Nitrous oxide abuse is increasingly common among young people. It's a gas consumed at parties, where access is easy and less expensive. It can cause vitamin deficiencies, leading to neurological and non-neurological clinical manifestations. The aim of this study was to establish the blood biology profile of patients hospitalized for neurological disorders in the context of nitrous oxide abuse. Methodology, we conducted a retrospective and prospective descriptive study from January 2021 to April 2023. We studied frequency of consumption, inhaled dose and time of exposure to nitrous oxide. Blood levels of vitamin B12 and B9 were measured. Methylmalonic acid and homocysteine were also measured in the blood. Results include 15 patients recruited during the study period. 7 of the 15 patients used nitrous oxide daily. One patient consumed 9600 grams per week. Ten patients had a delay in toxicity beyond one year. Vitamin B12 blood levels were normal in 8 of 15 patients. On average, vitamin B12 levels were normal at 207.57 pmol/l. Vitamin B9 was not routinely measured in 9 patients. Blood homocysteine levels were elevated in 12 of 13 patients, with a high mean of 83.36 µmol/l. In conclusion, blood homocysteine levels are a more reliable marker than vitamin B12 for demonstrating biological disturbances associated with laughing gas abuse.

Phytochemical Screening and Antioxidant Activity of Annona Senegalensis Extracts (Leaves and Stem Bark) Collected from Three Regions of Senegal

Abstract: The main objective of the present study is to focus on evaluation of the secondary metabolites composition and antioxidant activity of hydro – ethanolic extracts of Annona senegalensis leaves and stem bark which used in traditional medicine against several kind of diseases. The yields of hydro-ethanolic extracts of organs (leaves and Stem bark) were determined, followed by phytochemical screening and amount of secondary metabolites, by using conventional analytical methods. Anti-radical activity was carried out by DPPH (2, 2-diphenyl-l-picrylhydrazyl) reduction tests. The best extraction rates, whatever the plant organ or origin, were obtained with leaves (15.5% on average). Phytochemical screening revealed the presence of polyphenols, flavonoids and tannins. The highest levels of total polyphenols were obtained with leaves extracts [83.931 ± 0.018 g EGA/100 g Dry Matter (DM)] compared with stem bark (77.886 ± 1.001 g EGA/100 g DM). The highest tannin and flavonoid contents are 1.093 ± 0.006 g EC/100 g DM and 0.195 ± 0.004 g EC/100 g DM, respectively. Moreover, the extracts have a high antioxidant capacity with high inhibition percentages of 95.06 ± 0.10% for the leaves and 92.87 ± 0.10% for the bark. These results highlight the presence of bioactive compounds and potential antioxidant activity that have been shown to be efficient in the treatment of certain diseases, in accordance with ethno - botanical survey.

Association of Triglyceride/Glucose Index with CRP and Ferritin Among Different Obesity Phenotypes of Non-diabetic Adult Bangladeshi

Abstract: Introduction: Obesity is an alarming problem worldwide and it antedates insulin resistance (IR) as well as inflammation. Considering financial condition of Bangladesh, TyG index can be used for assessment of insulin resistance. As CRP and ferritin usually respond to any inflammatory condition, they are expected to be raised in obesity. Early detection of association between TyG index and inflammatory markers (CRP & ferritin) can help in prediction of severity of obesity induced health risks. Materials and Methods: A cross sectional analytical study was conducted in the Department of Biochemistry and Molecular Biology, (BSMMU) from March, 2022 to February, 2023. BMI was calculated, and individuals were classified into three obesity phenotypes: phenotype A (obese BMI, non-obese WC), phenotype B (non-obese BMI, obese WC), and phenotype C (obese BMI, obese WC). The Triglyceride-Glucose (TyG) index, serum ferritin, and CRP were assessed, and their correlations were analyzed across different obesity phenotypes. Results: TyG index was significantly higher in phenotype C compared to A & B. Plasma CRP and ferritin level were found to be highest in phenotype C in comparison to phenotype A and phenotype B. A moderate positive correlation was found between TyG index and serum ferritin but not with CRP in subjects belonging phenotype B. Very low positive correlation was found between CRP and ferritin in subjects of phenotype C. Conclusion: A significant association was shown between TyG index and serum ferritin among phenotype B. Plasma CRP showed no significant association with TyG index among different obesity phenotypes.
